My name is Nichole Forman, and I want to be your State Farm Agent! My team and I have a combined experience of over 30 years and our goal is to make sure you have the protection you need in case life does not go right.

I take pride in helping our customers and community prepare for the unexpected. If you are unsure of what coverages you have or don't have, please stop by with your policy and we will sit down and take a look with you.

Since opening in 2016, we have been dedicated to serving our community with a servant's heart. We are licensed in Texas, New Mexico and Oklahoma. Our physical office is in Lubbock, TX. We truly are a one stop shop for your auto, home, renters, business and life insurance!
